GeSY Contributions in 2026: Rates, the €180,000 Cap, and What You Get

· Demetris D.

GeSY, the General Healthcare System (in Greek ΓεΣΥ, internationally GHS), is the line on your Cyprus payslip that funds public healthcare. The rates have been stable since 2020 and remain unchanged under the 2026 tax reform. Here is exactly who pays what, and the two details that affect your net salary.

GeSY contribution rates in 2026

Who Rate Applied to
Employees 2.65% Gross salary
Employers 2.90% Each employee's gross salary
Self employed 4.00% Own income
Pensioners 2.65% Pension income
Income earners (rent, dividends, interest) 2.65% That income
The state 4.70% On all of the above

For an employee, only the 2.65% appears on the payslip. Your employer's 2.90% is paid on top of your salary and never touches your gross; it belongs in the true cost of employment.

The €180,000 cap

GeSY contributions are calculated on total annual income up to €180,000. Beyond that, no further GeSY is due for the year. On a €30,000 salary the contribution is €795 a year, €66 a month. On €200,000 it is 2.65% of €180,000, which is €4,770, regardless of how much higher the income goes.

GeSY reduces your income tax

The detail most people miss: GeSY contributions are deductible from taxable income, just like social insurance. The deduction order in 2026:

  1. Gross salary
  2. Minus social insurance (8.8%)
  3. Minus GeSY (2.65%)
  4. Income tax applies to what remains, under the 2026 brackets

On €30,000, the €795 of GeSY removes €795 from your taxable income. Since that income would have been taxed at 20%, GeSY effectively costs you €636, not €795. The salary calculator handles this ordering automatically.

What you get for it

GeSY gives every contributor access to the public healthcare system: personal doctors, specialists, labs, pharmacies, inpatient care, and since later phases also dentists and rehabilitation. There are small copayments per visit or prescription, with annual caps. Coverage is universal: contributing through any category, employment, self employment or passive income, brings you into the system.

GeSY for non employees, briefly

If you earn rent or dividends in Cyprus while also employed, those amounts attract their own 2.65% GeSY charge up to the same €180,000 combined cap. Self employed pay 4% on their income. Expats becoming tax residents join GeSY on the same terms; the expat tax guide covers how this combines with the 50% exemption.
